Otobe Supika is the daughter of the current head of the Otobe family, which has a great deal of influence over the government and military of Taishou-era Japan. She is frequently ill and generally lives believing that she will die sooner rather than later. She is very soft-spoken, but she has a mischievous side that primarily shows in Arima's presence.

She is a life-drinking human monster called an Iwaigo (blessed child) whose powers act very much like those of a succubus. As a child, she killed Arima's father and a number of others when she 'awakened' and indiscriminately 'ate' the life energy of all those around her. It was at this time (five years before the beginning of the story) that she raped Arima and he became responsible for 'handling her illness'.
